How do I apply?

Complete the application form in full. If you have been rejected for coverage by an insurance company, you must attach a copy of the rejection notice from the company. If you have received restrictive riders that substantially reduce coverage, you must provide proof of such restrictions, such as a copy of the insurance company's notice informing you of the rider.

Once your application is approved and we have received your first premium payment, we will send you an identification card, an insurance policy and a schedule of benefits. The insurance policy and schedule of benefits provide specific details of your plan's benefits and the procedures you need to follow in order to get the maximum benefits to which you are entitled.

You are encouraged to use the assistance of a licensed insurance agent when completing your application. ACHIA will pay a $100 fee to agents for these services.
